December 2018 Study Group Candidate Sign-Up
The deadline for signing up is July 13, 2018, at 5:00 pm.
- If you need to change your preferences AFTER submitting your form, simply SEND AN EMAIL directly to firstname.lastname@example.org indicating your requested changes.
ALL FORM FIELDS ARE REQUIRED, except for the Comments field box.
- You may choose only ONE Preferred and ONE Alternate Date/Time/Location
- You may indicate any flexibility in the “Comments” field box provided below.
- If your 1st and 2nd choices for one of the criteria (i.e., location, time or day) is the same in both instances, you must still choose that on the drop-down menu both times. Note: Please do NOT choose the same 3 criteria for both your 1st and 2nd choices.
Upon submitting this form, you will receive an email receipt, which will serve as your confirmation that we have received your form. NO FURTHER ACTION is required at this point.
Group Assignments Notification
The Membership Engagement Advisory Group will notify you regarding your specific study group assignment by July 20, 2018.
